• Form W-4—Employee’s Withholding Allowance Certificate (Adobe Acrobat)—Complete form W-4 so that Husson College can withhold the correct federal income tax from your pay. Because your tax situation may change, you may want to refigure your withholding each year.
• Form W-4ME—Employee’s Withholding Allowance Certificate (Maine) (Adobe Acrobat)—Complete form W-4ME so that Husson College can withhold the correct Maine income tax from your pay.
• Form I-9—Employment Eligibility Verification (Adobe Acrobat)—This form is required to establish your eligibility for employment in the United States. You will need to bring identification with you to the Career Development Office for this form to be processed. Acceptable types of identification are listed on the form.
• Direct Deposit Authorization (Microsoft Word)—Complete this form if you would like your pay directly deposited into your checking or savings account.
• Drug-Free Workplace Certification (Microsoft Word)—Husson College is proud to be a drug-free workplace. Please complete this form and return it to the Career Development Office with your W-4 and I-9.
